Each insurance company has it's own guidelines, but almost all have a network of dentists which you can contact directly, you should have been provided with a list of local dentists by your insurer.
You will have guidelines on how what procedures your plan entitles you to, for example 2 cleanings per calendar year. Most procedures such as fillings or simple gum treatments are straight forward and don't require preapproval by your insurer, but they will have provided an cost that if passed a quote would need to be submitted first for them to approve.
There will also be a yearly maximum cost, meaning that all procedures that you have during the year, whether minor or major, add up and cannot pass a set total cost.
